How to Make an Appointment

Request an Appointment Online

The best way to request a routine appointment online is to fill out the Econsult form.

This form will be reviewed by the GP within 2 working days and you will be contacted by a member of the team by phone or text with a plan for the most appropriate next steps.  This may be a call from the reception team to book a face-to-face appointment or telephone consultation with a GP, or a request to complete tests before seeing or speaking with a clinician (ie blood test, urine sample etc), or an appointment with our Nurse Practitioner or HCA.

Our doctors may also consult with you face-to-face, by phone, by video call or be text or email.

  • St. Helens Road – Telephone: 01204 665588 to make an appointment. The office is open daily between 08:30 –19:00 Monday – Friday.
  • At Southbrook – Telephone: 01204 363600. The office is open daily between 08:30 – 12:30. After 12:30 please ring the St. Helens Road surgery to make an appointment.

All doctors work at both surgeries; to book a Drs appointment please ring the surgery before 10am 

Emergencies & Enquiries

During normal surgery hours call: St. Helens Road on 665588 or Southbrook on 363600.

Telephone Consultations

If you wish to speak to the Doctor please ring the surgery and ask for a telephone consultation. These can be used for queries about your results, medications, sick notes or other minor illnesses.

Extended Hours

For patients who are unable to come to the surgery during normal surgery hours, we are now offering late night appointments at our Southbrook surgery from 6:30pm – 9pm for the doctor or practice nurses. Should this apply to you please contact reception. Please use this facility appropriately.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Patients who miss three appointments without letting us know may be removed from the practice list.

Home Visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling 01204 665588 before 10:00.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Late For Your Appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.
