New Patient Registration

We offer all new patients an initial health check with the healthcare assistants within the first six months. When you register with the Practice you will be allocated a named GP, but you may see which ever doctor you prefer.

There is an integral car park at Southbrook surgery and parking at St. Helens Road is on the main road adjacent to the front of the surgery. We have wheelchair access to both surgeries.

Practice Area

There is a boundary of area covered by the Practice to enable a better service for our patients. Please ask at reception to check if your address is within the area. If you move address outside the area covered you will be asked to register with a doctor’s surgery nearer to your new address.

Registration With The Practice

To register with the Practice you need to complete the registration form and the questionnaire for new patients listed below. When completed please bring to the surgery along with two forms of ID, one must be a photo ID and one must be proof of your current address.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
